Comments:
The Licensing Inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on January 27, 2020. The playground and equipment outside was inspected. Accident reports were reviewed. Staff records were reviewed. Interviews were conducted with staff relating to the allegations of roughly handling children, staff qualifications, accident report and outside playground equipment. The allegations of roughly handling children could not be validated. The information gathered during this investigation about playground equipment, staff qualifications and lack of accident reporting supports the allegations, so the complaint is determined to be valid. Standard #: 22VAC40-185-210-A-4
Complaint related: No
Description: Based on a review of staff records and staff interview, staff does not have six months of supervised programmatic experience and the required 24 hours of required training to meet the program lead qualifications.
Evidence: Staff record was reviewed for staff #2, hire date 1/13/2020. The record shows that staff #2 worked as an assistant at three other childcare centers. There is no documentation to show that staff # 2 is qualified for the program lead for the two's program. Staff #1 stated that she has the qualification.
Staff record was reviewed for staff # 3, hire date 2/4/2019. The record shows that staff has taken 17 hours of training. There is no documentation that staff #3 is qualified for the program lead for the toddler program. Staff #1 stated that she is taking educational classes.

Plan of Correction: I will get the documentation to show qualifications.

Standard #: 22VAC40-185-270-A
Complaint related: No
Description: Based on observation, areas and equipment of the center, inside and outside is not maintained in a safe and operable condition.
Evidence: The playground on the left is a Little Tykes picnic table. The bench seat has several cracks that could possibly injury a child.
The playground on the right is a Step 2 climber. There are several pink handles that are cracked exposing sharp edges. The second step of this same climber is cracked and not sturdy when stepped on.

Plan of Correction: I will remove the picnic table today. The climber will be removed Monday. Children will not be allowed on the climber today.

Complaint related: No
Description: Based on record review and staff interview, the center did not maintain a written record of a child's injury.
Evidence: Staff #1 was interviewed about the incident, staff stated that child #1 had scratches on the left cheek. Staff #1 showed the pictures that were taken on 1/13/2020 at 12:40 pm from her phone. There was multiple visible scratches on the child's left cheek that were thin and faint in color.
Staff # 4 was interviewed about the incident. Staff stated that the class went on a walk outside, she was holding child #1's left hand with her right hand. Staff #4 noticed the scratches and reported the injuries to staff #1.
Staff #1 and staff #4 stated an accident report was written and the parent was contacted.
An accident report has not been maintained at the center and could not be reviewed by the Licensing Inspector this date.

Plan of Correction: I will continue to maintain the accident reports.
