An unannounced monitoring inspection was conducted today between approximately 8:35am and 11:10am. 26 children were directly supervised by 5 staff members at the beginning. Ratios remained in compliance throughout the inspection. The physical space, 5 children's records, 3 new staff records, attendance, evacuation drills, medications and required posted documents were reviewed. Children were observed in circle time, working on age appropriate activities and interacting with the staff. Interaction were positive. Walls were decorated with children's artwork and developmentally appropriate materials and activities were available throughout. A new director was in place. Areas of non-compliance are identified in the violation notice.

Standard #: 22VAC40-185-270-A Description: Based on observation, areas of the center were not maintained in a clean, safe and operable manner.
Evidence: Equipment on the playground had multiple patches of rust, a pipe at the bottom of the fence that separated the playgrounds was broken and hanging to the ground, and a bolt was protruding from the base of the toilet in the bathroom across from the toddler room.Plan of Correction: I will put in a work order today to have everything fixed.
