Comments:
The licensing inspector conducted an unannounced complaint inspection on January 9, 2020 in response to a complaint received by our department on December 12, 2019. The inspector was on site from 4:05 PM to 5:50 PM. Interviews with staff were completed, observations were made and documentation was reviewed. The licensing inspector continued the investigation with a phone call on January 15, 2020. The allegations of the complaint were regarding ratios and supervision. Evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Violations that were not a part of the original complaint are listed on the violation notice. Description: Based on observation, it was determined that the areas and equipment of the center, inside and outside were not maintained in a safe condition. Evidence: The licensing inspector observed a raised corner of the carpet in the school age classroom that presented a tripping hazard.

Plan of Correction: Administration replaced the rug immediately.

Complaint related: No
Description: Based on observation, the center did not ensure that when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely. Evidence: The licensing inspector observed an infant to be seated in a high chair while drinking. The safety strap was not fastened.

Plan of Correction: Corrected on site.
