The licensing inspector continued the investigation with a phone call on January 15, 2020. The allegations of the complaint were regarding ratios and supervision. Evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Violations that were not a part of the original complaint are listed on the violation notice. If you have any questions about this inspection, please contact the licensing inspector at (804) 662-9092.
