Discussed record keeping and required trainings.

Comments:
An unannounced monitoring inspection was conducted between the hours of 1:00 pm-5:00 pm. At the time of entrance, 47 children were in care with 10 staff members present. Children were observed during snack time, handwashing, story time with letter recognition, and playing with toys. A selection of staff and children records, the physical space, evacuation drills, attendance records, injury reports, medications and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. A Subsidy Health and Safety Inspection (SHSI) was also completed on this date. Violations:
Standard #: 22VAC40-185-140-B
Description: Based on record review, one child's physical exam was not within the required time frame.

Evidence:
1. Child #4 began care on 7/1/19. The physical exam maintained was dated 1/9/18. The physical examination for this child was not obtained within 12 months of attendance.

Plan of Correction: I will request a new physical from the parents.

Standard #: 22VAC40-185-60-A
Description: Based on record review, 5 of 5 children's records did not contain all required information.

Evidence:
1. The record for Child #4 did not contain the work phone number for the father's place of employment and documentation of viewing proof of the child's identity and age.
2. The records for Child #1, 2, 3, and 5 did not contain documentation of the date the child's proof of identity and age was viewed.

Plan of Correction: I will ensure to have every portion of child's file filled out correctly. The parent was contacted during inspection.

Standard #: 22VAC40-185-240-A
Description: Based on record review, 2 of 4 staff did not receive all required training by the end of their first day.

Evidence:
There is no documentation of training on recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse in the record for Staff #1 (start date 9/17/18) and Staff #2 (start date 7/10/17).

Plan of Correction: I have emailed them the link to complete the training and will ensure all future employees complete the training before their start date.

Standard #: 22VAC40-185-550-D
Description: Based on record review, the center did not practice a minimum of two shelter-in-place practice drills per year.

Evidence:
1. A shelter in place drill was documented on 6/22/19. There was not documentation of a second shelter in place drill for 2019.

Plan of Correction: I will make sure to document drills when they happen.

Standard #: 22VAC40-185-550-M
Description: Based on record review, 1 of 3 injury reports did not contain all required information.

Evidence:
1. An injury report completed for Child #6 did not contain documentation of the time parents were notified.

Plan of Correction: I will follow up with staff members to make sure all parts of the form are complete.
