An unannounced monitoring inspection was conducted today from approximately 9:45 am to 12:00 pm. There were 47 children supervised by 10 direct care staff. The physical plant, 4 staff records, 5 children?s records, children's nutrition, evacuation drills, injury reports, emergency supplies were inspected. No medications were on-site today. The children were observed in free play, center activities, napping, and transitioning. There were age appropriate materials for all children. Staff-to-child ratios were in compliance.

Evidence: Child # 2, dated of enrollment 2/05/2019, did not have a completed physical examination available for review during the inspection.Plan of Correction: Emailed the parent and requested information.
Standard #: 22VAC40-185-160-A Description: Based on staff records review, 1 out 5 records, the center did not ensure that each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening.
Evidence: Staff #4's (D.O.E. 8/26/2019) did not have a negative tuberculosis screening in record available for review.Plan of Correction: Currently waiting on the results.
