An unannounced monitoring inspection was conducted on January 28, 2020 from approximately 3:30 pm to 5:30 pm. Two staff were present with thirty-seven children at the onset of inspection. An additional staff arrived during inspection. Four children records were reviewed. Staff training and information to be maintained on site were reviewed for three staff.

Standard #: 22VAC40-185-70-A Description: Based on interview and review, facility did not ensure 2 of 3 staff records has required documentation which shall be kept at the center.
Evidence: There was not documentation available at the center for staff #2 and staff #3 that includes name, address, verification of age requirement, job title, date of employment or volunteering and emergency contact information.Plan of Correction: Make sure all Prince George and Norfolk Champions employees information is at site in case they have to work at a location in an emergency.
Standard #: 22VAC40-185-260-A Description: Based on review, facility did not ensure there is an annual fire inspection report from the appropriate fire official having jurisdiction.
Evidence: The most recent fire inspection report available for review was dated April 5, 2017 which is more than a year old.Plan of Correction: Obtain a copy of the most recent fire inspection.
Standard #: 22VAC40-185-350-E-5 Description: Based on observation, review and interview, facility did not ensure there is at least one staff member for every eighteen children when the children are five through eight years.
Evidence: At approximately 3:30 pm, two staff were observed supervising one group with a total of thirty-seven school age children. Children ranged in age from five through ten years. Number of children present was confirmed on the written attendance sheet as well as by staff #1 and staff #2.Plan of Correction: Will make sure that employees be scheduled according to ratio.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation, facility did not ensure the findings of the most recent inspection is posted.
Evidence: The inspection notices posted were dated January 2019. The most recent inspection was conducted September 12, 2019.Plan of Correction: Will make sure updated inspection is posted immediately.
