An unannounced monitoring inspection was conducted on January 29, 2020 from approximately 7:00 am to 8:30 am. One staff was present with sixteen children at the onset of inspection. A second staff arrived during inspection. Five children and two staff records were reviewed.

Evidence: The record for child #4 (start date September 3, 2019) did not have an immunization record.Plan of Correction: Will obtain immunization record(s) from parents.
Standard #: 22VAC40-185-140-A Description: Based on review, facility did not ensure 2 of 5 children records has documentation of a physical examination within a month of beginning attendance.

Standard #: 22VAC40-185-260-A Description: Based on review, facility did not ensure there is an annual fire inspection report from the appropriate fire official having jurisdiction.
Evidence: The most recent fire inspection available for review was dated April 6, 2017 which is more than a year old.Plan of Correction: Obtain a copy of the most recent fire inspection.
Standard #: 22VAC40-185-340-C Description: Based on observation, facility did not ensure when one or more children are present that there is one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee with direct means for communication between the two of them.
Evidence: At approximately 7:00 am, staff #1 was present with sixteen school age children. Staff #1 stated staff #2 had left to go to another site to pick up paperwork. Staff #2 arrived to the facility between 7:10 - 7:15 am. A second staff or volunteer was not present during this time.Plan of Correction: Will make sure 2nd staff person is present.
