Comments:
An announced initial inspection was conducted today, February 3, 2020 to determine the facilities compliance with licensing standards. The inspection was initiated at 2:30 pm and concluded at 4:00 pm. The program's Director of Operations and the site director participated in the inspection. The facility plans to provide care to children ages four years, eleven months through twelve years eleven months and has requested a capacity of fifty children. The facility will operate in the Montrose Elementary School with care being provided in the cafeteria, library and gym. Children will have access to the schools playground equipment. Transportation will not be provided. The facility has age appropriate equipment and supplies. An afternoon snack will be provided to the children by the public school. Medications will be administered with proper authorization and according to the facilities policies and procedures. Standards related to supervision and program could not be determined as the facility is not yet fully operational. There were no children in enrolled at the time of the inspection. The records of two staff members were reviewed. The first aid kit and non-medical emergency supplies were reviewed. A conditional (6 month) license will be recommended. An unannounced follow-up inspection will be conducted within 60 days to determine compliance with standards not determined at the initial inspection.

Description: Based on review of the first aid kit, the center did not obtain all required items.

Evidence:

The first aid kit did not contain a first aid manual as required.

Plan of Correction: Per the administrator: I have additional first aid manuals at the office. I will provide one for the site's first aid kit.

Standard #: 22VAC40-185-540-E
Description: Based on review of the nonmedical emergency supplies, the center did not obtain a working, battery-operated radio in each building used by children.

Evidence:

1. The battery operated radio on site did not work.
2. The administrator acknowledged the battery operated radio did not work.

Plan of Correction: Per the administrator: I will purchase a new battery operated radio for the site.

Standard #: 22VAC40-185-550-A
Description: Based on interview and review of the emergency preparedness plan, the center did not develop the plan in consultation with local or state authories.

Evidence:

1. The emergency preparedness plan had not been developed with local or state officials.
2. The administrator stated the local officials had not yet completed reviewing the center's emergency preparedness plan.

Plan of Correction: Per the administrator: We are currently working with the local officials to review our emergency preparedness plan.
