Reminder: background checks are due every five years - please review as sworn disclosure statements and search of the central registry (CPS checks) will come due for some staff this year.
The water temperature should not exceed 120 degrees in sinks used by children. *Today's temperature read 120.9. so it may need to be reduced.
When assessing the injury prevention plan annually, please review the original plan to see if any updates are necessary.

Comments:
The licensing inspector conducted an unannounced monitoring inspection with the center director and center staff on January 29, 2020 to determine compliance with the licensing standards. There were 25 children present, ranging in age from 2 years to 5 years, with three staff actively supervising. The children were observed dancing, playing with puzzles, tracing letters/names, and preparing for lunch. Lunch served today: chicken noodle soup, cheese toast, peaches, applesauce and milk. Reportedly, mediation is not administered and transportation is not provided. Five children?s records and four staff records were reviewed - please see details on the Supplemental Summary. The last health inspection was conducted 4/24/2019. Pet immunizations were reviewed and are current. The last fire inspection was conducted 3/12/2019. Evacuation drills have been conducted monthly; the last shelter in place was conducted 10/24/2019. Violations:
Standard #: 22VAC40-185-270-A
Description: Description: Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: 1. A 6 inch kitchen knife was in a dish drying rack in the sink of the kitchenette, within reach of children, in the Four?s classroom. Several children were observed entering the kitchenette area to throw away trash.
2. The seat of a bike on the playground was torn, exposing approximately 4 inches of the inside foam/padding.

Plan of Correction: The knife is typically stored out of reach of the children. Staff will be reminded to keep it out of reach. The bike with the torn seat will be thrown away.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that hazardous substances be kept in a locked place using a safe locking method that prevents access by children. Evidence: A large bucket of floor glue was located under the bathroom sink in an unlocked cabinet, accessible to children. The glue contained warning labels that stated ?Keep Out of Reach of Children? and ?Warning.?

Plan of Correction: Corrected during inspection: the center director locked the floor glue.

Standard #: 22VAC40-185-550-M
Description: Based on review of injury reports, the facility failed to maintain all required information on written injury reports. Evidence: All injury reports since May 21, 2019 were reviewed. The injury reports did not include "any future action to prevent recurrence of the injury".

Plan of Correction: The future action to prevent recurrence will be added to future injury reports
