An unannounced record review inspection was conducted at the central location from 11:44am through 12:16pm. Four staff records were reviewed. Violations regarding documentation of staff qualifications, tuberculosis screening, annual training and background checks were identified and reviewed the site director at the conclusion of the record review.

Standard #: 22VAC40-185-160-A Description: Based on record review the center failed to ensure that each staff member has documentation of a negative tuberculosis screening submitted no later than 21 days after employment and completed within 12 months prior to or 21 days after employing or volunteering.
