Comments:
An unannounced record review inspection was conducted at the central location from 11:44am through 12:16pm. Four staff records were reviewed. Violations regarding documentation of staff qualifications, tuberculosis screening, annual training and background checks were identified and reviewed the site director at the conclusion of the record review.

Standard #: 22VAC40-185-160-A
Description: Based on record review the center failed to ensure that each staff member has documentation of a negative tuberculosis screening submitted no later than 21 days after employment and completed within 12 months prior to or 21 days after employing or volunteering.
Evidence:
1 - The record for staff 2 contains a tuberculosis that is dated March 22, 2018.
a. The documented date of hire for staff 2 is October 5, 2017.
b. The tuberculosis screening was obtained more than 21 days after the date of hire.

Description: Based on record review the center failed to ensure that that staff record contained written information to demonstrate that the individual possesses the education, orientation training, staff development, certification, and experience required by the job position.

a. The documented job title for staff one is program leader.

Description: Based on record review the center failed to ensure that staff who work directly with children annually attend 16 hours of staff development activities that are related to child safety and development and the function of the center.

a. The documented date of hire for staff 3 is August 28, 2018.
