Comments:
An unannounced record review inspection was conducted at the central location from 9:30am through 11:07pm. Four staff records were reviewed. Violations regarding documentation of staff qualifications and tuberculosis screening were identified and appear on the violation notice.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review the center failed to ensure each staff member obtains documentation of a negative tuberculosis screening within the allotted time frames.
Evidence:
1 - The record for staff 2 does not contain any documentation that a tuberculosis screening was completed.
a. Staff 2 has documented date of hire as January 29, 2019.
2 - The record for staff 3 does not contain any results of tuberculosis screening.
a. Staff 3 has documented date of hire as September 3, 2019.

Description: Based on record review the center failed to ensure that each staff record contains the required information.
Evidence:
1 - The record for staff 1 does not contain written information to demonstrate that the individual possesses the education required by the job position of program director.
a. Staff 1 was observed working in the position of program director during an onsite inspection on September 24th and 25th, 2019.
b. Staff 1 stated that he was the program director for the site during the inspection on September 24th and 25th, 2019.
c. Staff 1 has documented date of hire as August 22, 2018.
2 - The record for staff 2 states that staff 2 has a job title of program leader. However, there is not any documentation that staff 2 has obtained the required training to support this job title.
a. Staff 2 has documented date of hire as January 29, 2019.
3 - The record for staff 3 state the staff's job title is program leader. However, there is not any documentation that staff 3 obtained the required training to support this job title.
a. Staff 3 has a documented date of hire as September 3, 2019.
