Comments:
An unannounced renewal inspection was conducted on 2/4/20 from 10:25am until 11:55am. At the time of the tour, there were 31 children in care with 6 staff. A sample of 5 children's records and 5 staff/board member records were reviewed. Children were observed engaging in morning learning activities, playing on the playground and eating snack. Medication, the emergency preparedness plan, first aid and emergency supplies, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Two violations in the areas of administration and physical plant are identified on the violation notice and were discussed with the center director during the exit interview.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, the center did not ensure that staff records contain all the required information.

Evidence:
1. The record for staff 1 (date of hire: 2/1/17) contains documentation of only one reference as to character and reputation as well as competency were checked before employment, where two are required.
2. The center director confirmed that the record for staff 1 is lacking one of the two required reference checks.

Plan of Correction: The center responded with the following: This was an oversight. The center director will obtain a second reference check for staff 1, which will be documented in staff 1's record.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that hazardous substances, such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
There was a can of Lysol disinfecting spray on an open shelf above the sink in the pre-k classroom. The container was labeled "keep out of reach of children" and "caution: flammable" and was accessible to the children in care.

Plan of Correction: The center responded with the following: The can of Lysol was placed in a locked cabinet during the inspection.
