Technical Assistance:
All staff shall completed the Preservice Update Training no later than 3/1/20.

Discussed better options for the large mat that is in place at the bottom of the stairs on the playground. The Inspector suggested an angled mat be purchased and installed there, for improved tripping safety.

Comments:
An unannounced monitoring inspection was conducted today. There were 52 children in care, with a total of 11 staff members assigned to their direct supervision. Children's records, staff records, staff training, background checks, administrative records, emergency preparedness items, the physical plant, classroom programming/interactions, and children's medications were inspected today.

The children were observed playing outside, attending chapel, playing in the gym, enjoying a wide variety of classroom activities, coloring/painting pictures, and eating lunch.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that five records do not include all required information.
Evidence:
1 - Child record A does not include: the parent's home phone number; documentation that the parent has reviewed the child's contact information within the past year.
2 - Child record B does not include: the parent's home phone number; the mother's work phone number.
3 - Child record C does not include: "authorized pick ups" information; documentation that the parent has reviewed their child's contact information within the past year.
4 - Child record D does not include: documentation that the parent has reviewed their child's contact information within the past year.
5 - Child record E does not include: the parent's home phone number.

Plan of Correction: All parents will be asked for the missing information.

Standard #: 22VAC40-185-280-B
Description: Based on observations, it was determined that two potentially hazardous substances were not kept in a locked place.
Evidence:
Containers of table cleaning wipes were stored inside unlocked cabinets in the Wise Owls classroom and the Busy Bees classroom.

Plan of Correction: They will be locked today.

Standard #: 22VAC40-185-280-G
Description: Based on observations, it was determined that two substitute containers of potentially hazardous substances were not labeled to indicate the contents.
Evidence:
In the Teddy Bears classroom and the Busy Bees classroom, the spray bottles of bleach/water cleaning solution were not labeled.

Plan of Correction: They will be labeled today.

Standard #: 22VAC40-185-510-A
Description: Based on a review of children's medications, it was determined that school has not obtained written authorization for one child's long term medication.
Evidence:
The authorization form for one child's long term emergency medication has not been signed by the parent.

Plan of Correction: I will ask the parent to sign the form today.
