Technical Assistance:
The following was discussed with the program director:
Cleanliness of the classrooms and regular changing of air filters.

Comments:
An unannounced monitoring inspection was conducted between the hours of 9:00 am and 1:00 pm. Upon arrival there were 99 children in care with 20 staff present. A sample of 2 staff and 9 children's records were reviewed, in addition to 2 medications. There was one child in care with a documented food allergy. Children were observed playing in centers, on the playground and eating lunch. Infants were observed sleeping in cribs, in bouncers and having tummy time. Diapering was also observed.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
There were large wooden cubbies throughout the center that were not secured and posed a tipping hazard.

Plan of Correction: The following plan of correction was provided by the director during the exit interview:
The director will secure all wooden cubbies within the center.

Standard #: 22VAC40-185-280-B
Description: Based on observation the center did not ensure hazardous substances were kept in a locked place.
Evidence:
In the yellow classroom (school agers) there was an unlocked cabinet containing a spray bottle of bleach water and a bottle of disinfectant spray.

Plan of Correction: The following plan of correction was provided by the director during the exit interview:
The director will remind staff about labeling all chemicals and all cabinets are to remain locked.

Standard #: 22VAC40-185-280-F
Description: Based on observation the center did not ensure when hazardous substances are kept in original containers, they were clearly labeled with its contents.
Evidence
In the yellow room (school-agers) there was a spray bottle in an unlocked cabinet filled with a liquid. The bottle was not labeled in anyway that stated its contents.

Plan of Correction: The following plan of correction was provided by the director during the exit interview:
The director will remove the spray bottle and will determine its contents. All staff will be reminded about labeling all chemicals and all cabinets are to remain locked.

Standard #: 22VAC40-185-500-B
Description: Based on observation the center did not ensure all requirements were met when diapering.
Evidence:
A staff member in Red 2 class (toddlers) was observed changing a child's diaper. Once the diaper change was complete the staff member placed the child on the ground. The staff member did not wash her hands until after observed opening a wipes container, opening a cabinet to obtain cleaning products and then cleaning the diapering surface.

Plan of Correction: The following plan of correction was provided by the director during the exit interview:
The director will meet with the staff and retrain on diapering and have her sign-off on the training.
