Technical Assistance:
Technical assistance was provided regarding staff record organization.

Comments:
An unannounced renewal inspection was conducted today from 10:00am-2:00pm. There were 73 children supervised by 9 direct care staff. The physical plant, 6 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, eating lunch and nap time. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Description: Based on review of six staff records four do not have complete reference check information documented.
Evidence:
Staff #1 and #3 do not have documentation of who checked their two references.
Staff # 2 and #4 do not have documentation of any references.

Plan of Correction: I will go through all the records to make sure they are complete.

Standard #: 22VAC40-185-240-C
Description: Based on review of six staff records, five are incomplete and do not have documentation of 16 hours of annual training related to child safety and development.
Evidence:
Staff # 1, 2, 3, 4, 5, do not have documentation of completing 16 hours of annual training.

Plan of Correction: We will provide certificates for all staff attending training's and keep in their record.

Standard #: 22VAC40-185-270-A
Description: Areas of the center not be maintained in a clean, safe, and operable condition.
Evidence:
Chipped paint under the window in the PreK 2 room is accessible to children.
Two children's chairs,located in the PreK 2 room, are ripped and torn with the stuffing coming.

Plan of Correction: Rooms are being painted by the 14th.
Chairs were removed from the classroom and will be fixed before going back in the classroom

Standard #: 22VAC40-185-510-N
Description: Based on review of six medications, two do not have documentation of current medication authorization forms.
Evidence:
Child #1 has two medications on site with medication authorizations that expired in October 2019.

Plan of Correction: You will contact the parent and get new authorization forms completed.
