Technical Assistance:
Please consistently use either the MAT or DSS medication form and follow the time line for authorizations.

Comments:
An unannounced monitoring visit was conducted. All rooms were in ratio with direct care staff. Observed children in circle time, infant care, resting, transition to outside playground time, diapering and bottle feeding. Emergency preparation is current. Reviewed newly hired staff records, drop-in care children's records, medication and nutrition. Lunch served today was chicken chili, broccoli and cheese salad, bread and butter and fruit. Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review, 2 of 7 children's records were missing additional immunization documentation.
Evidence:
Child #3 (last update 4/2019) and Child #5 (last update 5/19) did not have updated immunization records every 6 months for children under two years of age.

Plan of Correction: These were backup children. We will monitor those files closely and collect all current missing information.

Standard #: 22VAC40-185-60-A
Description: Based on record review, 4 of 7 children's records did not contain all required information.
Evidence:
Child #3 was missing 2 emergency contact addresses
Child #4 was missing an emergency contact
Child #5 was missing proof of identity
Child #7 was missing 2 emergency contact addresses

Plan of Correction: These were backup children. We will ensure the children are not dropped off without proper paperwork. We will collect all missing information.

Standard #: 22VAC40-185-240-A
Description: Based on record review, documentation that a staff received orientation training on the first day of assuming job responsibilities was not available.
Evidence:
Staff #2 (date of hire 1/6/2020) did not have orientation training documentation in the personnel record.

Plan of Correction: The orientation checklist was in a different spot but was completed on time. We will ensure all paperwork is in the correct employee file.

Standard #: 22VAC40-185-270-A
Description: Based on observation, an area of the center was not maintained in a clean, safe or operable condition.
Evidence:
1.The door frame of Infant I classroom was peeling paint in one spot.
2. The light blue rug in Preschool 1 was unraveling in one spot.

Plan of Correction: The door framw was repainted on 1/31/2020. The string was cut off the rug in preschool 1 that day.

Standard #: 22VAC40-185-280-B
Description: Based on observation, bottles containing cleaning/bleach solutions were not kept in a locked place using a safe locking methods that prevents access by children.
Bleach bottles were stored in unlocked upper cabinets in classrooms Early Preschool 2, Preschool 1 and the bathroom between Early Preschool 2 and 3.

Plan of Correction: Bleach bottles were out of children's reach but not locked. More cabinet locks were purchased and applied to those doors.
