An unannounced renewal inspection was conducted on 02/06/2020. There were 19 children, ages 5 months-2 years, and 7 staff members present during the inspection. The inspector reviewed 5 children's records, 6 staff records and 1 medication during the inspection. The children were observed in the following activities: in free choice activities, in music and movement, during story time, having lunch and during nap time. Infants were observed being allowed to sleep and being fed on demand. The inspector reviewed the parent and staff policies for the facility at the main branch. The inspector reviewed the Injury Prevention Plan and the emergency preparedness plans and the emergency supplies during the inspection.The inspector discussed the following with program staff: product recall information, children's allergies, transportation and topical ointments.The inspector arrived for the inspection at 9:00 a.m. and departed at 11:45 a.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Standard #: 22VAC40-185-520-C Description: Based on document review and discussion with the staff, the facility failed to ensure that when diaper ointment or cream is used, written parent authorization noting any known adverse reactions shall be obtained.
Evidence: A medication authorization form was used for parent authorization to apply diaper ointment; the medication authorization forms do not note any known adverse reactions. There was no documentation of any known adverse reactions for any of the diaper ointments on-site.Plan of Correction: The model form for over the counter skin products will be used.
