Comments:
An unannounced monitoring inspection was conducted November 25 2019 between 3:00pm and 5:00pm. There were 13 children in the direct care of 2 staff members. Children were observed eating snack, washing hands, completing homework, and playing outside. Children's records were reviewed and the first aid kit, medication, and physical plant was inspected. Description: Based on record review, 1 of 5 children's records reviewed did not contain documentation of all of the required information.

Evidence: Child # 1 record did not have documentation of 2 complete emergency contacts including addresses and telephone numbers. Only 1 contact was listed.

Description: Based on observation, the center did not have a posting the most recent violation notice.

Evidence: The center had a violation notice posted that was not the most recent. The violation notice was dated 12/4/2018 and the most recent inspection that had taken place was dated 4/25/2019.
