Comments:
An unannounced monitoring visit and facility tour was conducted from 12:20 pm-2:00 pm. At the time of inspectors' arrival there were 6 preschool aged children in care with 1 staff member and 1 administrator. Children were observed participating in instructional time, and transitioning between activities. A sample of 5 children's records and 4 staff records were reviewed. Injury reports were also reviewed. Areas of noncompliance are detailed on the violation notice and were discussed with the Director at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-240-C
Description: Based on record review and interview it was determined the center did not ensure all staff who work directly with children annually attend 16 hours of staff development activities that shall be related to child safety and development and the function of the center.
Evidence:
1. The record for staff #2(hired 7/11/16) did not contain documentation of the 16 hours of required annual training. There was only 4 hours of training documented for July 2018-July 2019.
2. Staff #1 confirmed staff #2 did not complete the required annual training hours.

Plan of Correction: Staff will ensure that all staff will complete the required annual training hours.

Standard #: 22VAC40-185-260-A
Description: Based on record review it was determined the center could not provide the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.
Evidence:
1. There was no current fire inspection. The current fire inspection was dated 11/27/18.
2. Staff #1 confirmed that there was no updated fire inspection available.

Plan of Correction: Director will call to request an updated fire inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
In the boys rest room there was peeling paint behind the toilet and a small 1/16" gap between the switch plate cover and the wall.

Plan of Correction: Director will ensure that repairs are made.
