Technical Assistance:
Clarification given regarding clear documentation of injury reports.

Comments:
An unannounced Monitoring Inspection was conducted on February 6, 2020, from approximately 10:15 am until 1:20 pm. Upon arrival there were 34 children in care 6 with staff. Additional staff were present for administrative purposes. Infants were observed eating lunch, freely playing, working on puzzles and coloring. Children were observed playing in centers to include blocks, cars, dinosaurs and during the diapering process.
The sample size of records reviewed consisted of 5 children's records and 3 staff records. The center is not administering medication. The playground was not inspected due to inclement weather. Violations from the previous inspection were reviewed.
The results of the inspection were reviewed and verified with the center owner during the exit interview. Areas of non-compliance are noted on the violation notice. Please contact the licensing inspector, Christine Mahan (757) 404-0568 with any questions.

Violations:
Standard #: 22VAC40-185-500-B
Description: Based on observation and staff interviews, the licensee did not ensure the diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use.

Evidence: Observation was made of the diapering process in the two's and three's room. After diapering child #6, the center staff did not use soap and water to clean the diaper pad. The staff member only sprayed the diaper pad with a bleach and water solution and then immediately wiped it down with a paper towel and it was not left to air dry. The diaper pad was not cleaned and was not sanitized.

Plan of Correction: The center owner will retrain the staff and go over proper steps of cleaning and sanitizing

Description: Based on observation and staff interviews, the licensee did not ensure tables and high chair trays shall be sanitized before and after each use for feeding.

Evidence: Four infants were observed eating lunch in highchairs and the highchairs were not sanitized prior to eating. The center staff in the infant room confirmed the highchair trays were only wiped with a baby wipe. Sanitized" means treated in such a way to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution. The surface of item is sprayed or dipped into the disinfectant solution and allowed to air dry after use of the disinfectant solution.

Plan of Correction: The center owner will retrain the staff and go over proper steps of cleaning and sanitizing.
