Comments:
An unannounced monitoring inspection took place on February 3, 2020. There were 28 children in care from ages three years old to five years old with four staff providing care and supervision. The inspector reviewed five children's records onsite and five staff records at another location, on January 6, 2020. The inspector reviewed all medications at the center, which was one medication total. This center states that it does not transport children. Children were observed during the following activities: outside time, hygiene practices, lunch, story time, bingo, and nap. The inspector also reviewed emergency drill documentation, first aid kits,physical plant safety, and cleaning/sanitizing agents. There were discussions about children's records and the playground. The inspection took place from approximately 11:15 AM to 1:45 PM. Violations were discussed at the conclusion of the inspection. Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that each center shall maintain and keep at the center, a separate record for each child enrolled which shall contain all the elements as required by the standards.

Evidence:

1. The record for Child 2 did not contain information regarding allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation; the record also did not contain chronic physical problems, pertinent developmental information, and any special accommodations needed.
2. The record for Child 2 did not contain a complete phone number for a second contact to call in an emergency if a parent cannot be reached; the phone number only contained six digits.
3. The record for Child 5 did not contain health information as required by 22VAC40-185-150, a physical examination. The child's date of enrollment was 11/25/2019.

Description: Based on observation and discussion with staff, the center failed to ensure that if hazardous substances are not kept in original containers, the substitute containers shall clearly indicate their contents and shall not resemble food or beverage containers.

Evidence:

A staff member removed a spray bottle from a cabinet. The bottle was not labeled The inspector asked what chemicals were inside the spray bottle. Staff said that the contents were bleach and water. Another staff member confirmed that the substitute container had recently been replaced and not labeled "bleach and water."
