Comments:
A Subsidy Health and Safety inspection was conducted on 1/28/20 from 2:55pm - 4:50pm. During the inspection there were 135 school age children in care with 12 staff. Children were observed participating in various activities in the classrooms and doing homework. Records were reviewed for ten children while at the facility. The records for ten staff were reviewed on 1/31/20 at the central office. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-580-E-1
Description: Based a review of ten staff records, it was determined that the vendor did not ensure that all staff have a current certification in cardiopulmonary resuscitation (CPR) and first aid appropriate to the age of children in care.

Evidence:
1. The record for staff #3 did not contain documentation for a current certification in cardiopulmonary resuscitation (CPR) and first aid appropriate to the age of children in care.
2. Staff #11 reviewed the record for staff #3, and confirmed that there was not documentation to demonstrate that staff #3 had a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care.

Plan of Correction: The vendor responded: Staff #3 has completed a course to obtain certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care, but we were unable to locate it during the inspection. We will obtain a copy of the certification and place in the record for staff #3. All new staff will complete a course to obtain certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care within 90 days of employment.
