Comments:
Thank you for your assistance and your time during this unannounced monitoring inspection. The visit was conducted on January 24, 2020 from 9:20 am through 12:10 pm. There were fifty-one children in the care of seven staff. The total enrollment for the program was sixty-eight children. There were five children's records, six staff's records, medication and documentation for one child, evacuation drills, emergency supplies, health inspection, and fire inspection reviewed. Lunches and snacks were provided by the children?s parents. Children?s work cycle, diapering procedures, hand washing, bathroom procedures, snack, reading, outside play, staff/child interactions, daily schedule, transitions, art, and lunch were observed during this visit. The exceptions to compliance were noted on the violation notice.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of six staff records and verification from the administrator, the center failed to ensure a new staff member obtained documentation of a tuberculosis (TB) screening no later than 21 days after employment.

Evidence:
1. The TB screening results for staff #1's record is dated 01/09/2020. The first date of employment for staff #1 is documented in the record as 12/10/2019.
2. The administrator verified that staff #1's TB screening was completed after 21 days of employment.

Plan of Correction: Spoke with administrator not to allow this to occur.

Standard #: 22VAC40-185-500-B
Description: Based on observation and verification from a staff member, the facility did not ensure that a nonabsorbent surface for diapering or changing was used.

Evidence:
1. The inspector observed tears in the surface that covers the pad used to change diapers in the Pre-primary classroom. The tears exposed an absorbent sponge.
2. Staff member confirmed that the mat they were using had absorbent sponge exposed.

Plan of Correction: We will begin using a non-absorbent mat/pad.

Standard #: 22VAC40-185-510-E
Description: Based on review of medication for one child, the center failed to ensure that prescription medication was labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.

Evidence:
The medication for Child D was not labeled with child's name, name of medication, dosage, and times to be administered.

Plan of Correction: The medication was labeled, had the medication name on it, and it was a single dosage. However, the original prescription box was missing. A replacement has been requested from the parents.
