Comments:
An investigation was begun in response to a self reported incident being received in our office. See violation notice for non-compliance.

Violations:
Standard #: 22VAC40-185-340-A
Description: Based on multiple staff interviews, the facility did not ensure care protection and guidance for one child in care.
Evidence:

On 1/21/2020, a staff person observed Staff A "pinch" a child they were holding in the Infant classroom, and then heard the child scream. An additional staff person in the room, heard a door slam and then the child scream.
The incident was not immediately reported to administration; however, when the child was checked the next morning, there was a mark on the child's stomach.

Plan of Correction: There will be immediate retraining on positive guidance as well as notifying administration of anything that makes them uncomfortable.

Standard #: 22VAC40-185-410-1
Description: Based on multiple staff statements, behavior guidance was not constructive in nature.
Evidence:
On 1/21/2020 a staff person was observed "pinching" a child that they were holding. The child was "fussy" and had been crying, and was picked up to be held by the staff person. The child screamed after being pinched. The child was then placed on a cot to rest.

Plan of Correction: There will be immediate retraining on positive guidance. Administration will increase room visits for observations.

Standard #: 22VAC40-185-550-L
Description: Based on staff statements, no injury report was completed for a child that sustained an injury at the facility.
Evidence:
A child was pinched by a staff member, leaving a mark on their stomach on 1/21/2020. No report was completed even after the facility was made aware of the injury. Parents were not notified by the end of the day on 1/21/2020. Parents were called on 1/22/2020 to inform them of the incident.

Plan of Correction: Injury reports will be completed the day or as soon as possible.
