Comments:
An unannounced monitoring inspection was conducted between the hours of 10:00 am and 12:00pm. Upon arrival there were 73 children in care with 19 staff present. A sample of 4 staff and 7 children's records were reviewed. Children were observed in learning centers and on the playground. Infants were observed in cribs, being fed and on the floor interacting with staff.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
1. In bathroom shared by the twos and preschool class, three of the toilets had rusted metal plates connecting the plumbing to the wall.The metal plates were in reach of children.
2. In the preschool room the center had a large Prowise tablet Board that was in the upright position and did not have the safety locks on. Due to its size, not being secured and safety locks not in use, it posed a tipping hazard.

Plan of Correction: The following corrective action was provided by the District Manager (DM): The school is working in conjunction with our facilities team to ensure that the bathroom plates will be replaced on or before 3/6/20. In Pre K the staffwill be retrained on ensuring that the safety locks will be used at all times. Management will observe on a regular basis for compliance.

Standard #: 22VAC40-185-500-A
Description: Based on observation the center did not ensure all handwashing requirements were met.
Evidence:
In the toddler room a staff member was observed changing a child's diaper. After the diaper change, the staff did not wash her hands until after the following steps: opened the cabinet, took out cleaning products and cleaned the diapering surface. At the time the child was taken down from the changing table, staff told the child to throw away a wipe she had in her hand. The child used her hands to open the foot operated garbage can and placed the wipe inside. The child's hands were not washed.

Plan of Correction: The following corrective action was provided by the District Manager (DM):
Toddler Staff will be retrained on or before 2/10/20 regarding all proper sanitation and handwashing procedures when diapering. Management will observe weekly for compliance

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a record review the center did not ensure all staff had the required background checks.
Evidence:
Staff #3 (hired 11/04/19) indicated on her sworn statement, living outside the state of Virginia within the past 5 years. The center did not have documentation of the results of a central registry check in that state. In addition, the center did not have any documentation that the background check was requested from the state.

Plan of Correction: The following corrective action was provided by the District Manager (DM): Effective immediately, management will send the DM a copy of the sworn disclosure for all new hires, prior to requesting a hiring approval, to verify out-of-state residence and the completion of any applicable background checks.
