63.2(17) License & Registration Procedures

Comments:
An unannounced non-mandated monitoring inspection was completed on 2/11/2020. There were 87 children in care with 19 staff. The inspector arrived at 7 A.M. and departed the program at 11:45 A.M. The inspector reviewed seven staff's and children's records. Ten medications were reviewed. Nine classrooms and the gym was observed in use.

Observed the children during: arrival into the program, circle time, hand washing and toileting procedures morning snack and gym time.

Discussion with director about licensing renewal process.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of seven children's records, the center failed to ensure that all physical examination were obtained with in the 30 days of enrollment.

Evidence:
1. Child # 2 started on 9/18/2019 and on the day of the inspection there was no documentation of the physical examination.

Plan of Correction: The assistant director stated that she sent the physical back with the parents to obtain a signature. The parents will be notified to return a signed copy as soon as possible.

Standard #: 22VAC40-185-160-A
Description: Based on review of seven staff's records, the center failed to ensure that all TB screening were completed within the first 21 days of hire.

Evidence:
1. Staff # 4 started on 9/19/2019 the TB screening was completed on 1/5/2020.

Plan of Correction: The assistant director will make sure that all TB screenings are within the required 21 days of enrollment.

Standard #: 22VAC40-185-510-G
Description: Based on review of medications, the center failed to ensure that medications were administered as required by the manufacturers recommendation.

Evidence:
1. Child # 2 was given infant Tylenol on 1/22/2020, 1/23/2020 and 1/29/2020. The manufacturers recommendation for dosage is to consult a physician for children under the age of two years. The children was 23 months old as the time when the medications was administered.

Plan of Correction: The assistant director will return the medication to the parent today. In the future all medication will be monitored for manufacturer recommendations.

Standard #: 22VAC40-185-510-N
Description: Based on review of medications, the center failed to ensure that all medications was returned to the parent within the 14 days after the authorization was completed.

Evidence:
1. Child # 8 authorization form was completed on 1/22/2020 and was not returned to parent after the authorization to administer the medication had expired on 2/03/2020.

Plan of Correction: The medication will be sent home with the parent today. In the future all medication will be monitored.

Standard #: 22VAC40-185-560-N
Description: Based on observation, the center failed to ensure that children were not walking around while drinking or eating.

Evidence:
1. Observed the two children in the gym area walking around while drinking from their sippy cups.

Plan of Correction: The director reminded the substitute teacher that children are to sit down while drinking and removed the cups.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of seven staff's records, the center failed to ensure that a returned finger print eligibility letter was received prior to starting.

Evidence:
1. Staff # 4 was hired on 9/19/2019 and the date on the finger print letter of eligibility letter was received on 1/3/2020.

Plan of Correction: In the future the administration will make sure that finger prints are returned prior to starting position and or paperwork.
