Comments:
An unannounced monitoring inspection was conducted between the hours of 10:00 am and 12:00 pm. A sample of 4 children and 3 staff records were reviewed. Preschool children were observed in learning centers, during lunch and toileting. Infants were observed sleeping in cribs, engaging in tummy time and on the floor with staff.

Violations:
Standard #: 22VAC40-185-400-A
Description: Based on observation and interview the center did not ensure staff interacted with children in order to provide needed help, comfort and support.
Evidence: A group of 3 year-old children were observed seated and about to eat lunch. Child #1's face was flushed and wet and crying at the table. The child was observed placing her head in a blanket she had on her lap a few times. There were 3 staff in the room, each performing a task. One of the staff addressed child #1 and stated. "You're okay," however did not provide any comfort to the crying child. When the inspector asked why the child was crying, the staff said it was because she is not usually in this room, but was moved to maintain ratio.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
The assistant director went into the classroom during the inspection to address the situation and speak to the staff and comfort the crying child. Staff were provided with a counseling form to address positive behavioral guidance with the children. A staff meeting is being held on Monday. Behavioral guidance will be discussed and guidance will be provided to the staff.

Standard #: 22VAC40-191-60-C-2
Description: Based on a record review the center did not ensure all background check requirements were met.
Evidence:
Staff #1, hired on 08/01/19, did not have the results of a central registry check within 30 days of employment. Staff #1 was working with children the day of the inspection.

Plan of Correction: The following plan of correction was provided by the director during the exit interview:
The check was sent, however the results had not come back yet. This was an administrative oversight. The Director called the Office of Background Check Investigations during the inspection to follow-up on staff #1's background check results. As soon as the results are sent to the center, a copy will be forwarded to the inspector.
