Comments:
An unannounced Renewal Inspection was conducted from 3:05 p.m. to 4:55 p.m. on 1/24/2020. At the time of entrance 8 children, ages 5 to 9 years old were in care with one staff. The records sample size consisted of 5 children?s records. Two staff records were reviewed during a record review at the AlphaBEST Area Managers? office on 2/3/2020 and 2/4/2020.
Children were observed creating crafts, socializing, and playing table top games / activities. Staff stated that no medication is being administered to children in care at this time and that children are not transported in a vehicle while in care, Thank you for your time during today's inspection. Violations:
Standard #: 22VAC40-185-340-C
Description: Based on observation and interview, the licensee did not ensure that during the stated hours of operation when there are children in care there is always present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them.
Evidence:
At the time of arrival, the inspector observed 8 children with one staff member in the gymnasium.
When asked if there were more children present with another staff, staff #2 stated that there were no other children or staff present on site. Staff #2 explained that she normally has a substitute staff present to assist her but that she had been unable to be there on this date.

Plan of Correction: The area manager stated that a candidate has been selected for this site and paperwork is being processed in preparation for the staff to be trained and hired. Until then, a substitute will be scheduled to work with staff #2 at this site.
