Comments:
An unannounced Renewal Inspection was conducted from 4:20 p.m. to 5:45 p.m. on 2/7/2020. At the time of entrance 4 children, were in care with 2 staff. During the inspection 2 additional children arrived. The ages of children present were 6 to 12 years old The records sample size consisted of 8 children?s records. Two staff records were reviewed during a record review at the AlphaBEST Area Managers? office on 2/3/2020 and 2/4/2020.
Children were observed socializing, reading, and playing table top games / activities. Staff stated that no medication is being administered to children in care at this time and that children are not transported in a vehicle while in care. Repeat violations were found and are identified within this report. Thank you for your time during today's inspection. Please do not hesitate to contact me if I may be of assistance.

Standard #: 22VAC40-185-60-A
Description: Based on a review of records and interview, the licensee did not ensure that each enrolled child has a record containing all of the required documentation.

Evidence:
Eight children's records were reviewed.
The records for child #5, #6 and child #8 did not contain the addresses for the two designated people to call in an emergency if a parent cannot be reached.
When asked, staff #1 confirmed that she was also unable to locate this information in the record.

Plan of Correction: Staff #1 stated that this information would be obtained from the parents at the time of pick up and documented in the children's records.

Standard #: 22VAC40-185-70-A
Description: Based on a review of records, the licensee did not keep a record for each staff person which contained all required documentation.
Evidence:
Five staff Records were reviewed.
The record for staff #2 contained documentation of only one of the two required references as to character and reputation as well as competency were checked before employment or volunteering.

The area manager was unable to locate documentation of a second reference for staff #2 during the inspection.

Plan of Correction: The area manager will try to locate this documentation or ask that new references be obtained and place documentation of such in staff #2's record.

Standard #: 22VAC40-185-240-D-5
Description: Based on a review of records and interview, the licensee did not ensure that there is always at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.
Evidence:
The records for all (2) staff regularly assigned to this before and after school site were reviewed in the Area Managers' office on 2/3/2020 and 2/4/2020.
Neither of the two (#1 and #2) staff records contained documentation of staff having been trained in performing the daily health observation of children.
When asked if the staff had obtained Daily Health Observation Training, the area manager stated that this training had not yet been obtained but was already scheduled for 2/14/2020.

Plan of Correction: Staff #1 and #2 will obtain Daily Health Observation training on 2/14/2020 and documentation of the training will be placed in the staff records at that time

Standard #: 22VAC40-185-260-B
Description: Based upon observation and interview, the licensee was unable to provide to the licensing representative an annual health inspection report from the appropriate health official having jurisdiction.
Evidence:
When the annual health inspection documentation was requested of staff, they were unable to locate or obtain a copy of this documentation during the inspection.

Plan of Correction: Staff #1 stated that she would communicate with the school office to try to obtain a copy of the documentation of the most recent Health Inspection conducted of the school.

Standard #: 22VAC40-80-120-E-2
Description: Based upon observation and interview, the licensee did not ensure that the findings of the most recent licensing inspection of the facility were posted visibly.
Evidence:
The inspector did not observe posted in the facility the most recent inspection results from the last licensing inspection which was conducted on 10/23/and 10/24/2019.
When asked where the inspection results were posted, staff stated that they did not have a copy of the last inspection to post.

Plan of Correction: Staff stated that they will follow up with their supervisor if a copy of the current inspection is not sent to them to ensure that they have a copy to visibly post.
