Comments:
An unannounced monitoring inspection was conducted today from 9:20 am-10:50 am. There were 23 children present directly supervised by 6 staff and co-op parents. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in circle time to include: pattern finding, show and tell, the pledge of allegiance, and calendar work. Children were also engaged in sensory play and making Valentine's Day crafts. Hand-washing procedures were also observed. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-60-A
Description: Based on record review, one of five children's records reviewed was incomplete.
Evidence:
Child 5's record did not contain street addresses for two of the listed emergency contacts.

Plan of Correction: We will ask the parents to supply the missing information.

Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas of the center were maintained in a safe and operable manner.
Evidence:
1. A small bookcase in the upstairs red and blue classroom has a chunk of the particle board which is missing. The bookcase in its current state has exposed particle board and an exposed screw.
2. Peeling paint was observed on the wall in the upstairs red and blue classroom.

Plan of Correction: We will repair the items.

Standard #: 22VAC40-185-510-C
Description: Based on a review of medication, five of five medications reviewed did not meet the requirements for child day center's administering medication.
Evidence:
The medications for Children 1, 2, 3, and 4 did not contain a length of duration on the authorization form.
2. The long term medication authorization for child 4 did not contain the dose of the medication to be administered, the strength of the medication to be administered, and a physician's signature.

Plan of Correction: We will update the forms to include a duration date and will ask the parents to complete all missing information.
