documenting all accidents/incidents with infants, mandated reporting, ensuring that the water temperature in all sinks is no hotter than 120 degrees Farenheit

Comments:
An unannounced monitoring inspection was conducted on 12/31/19 and 1/9/20, in conjunction with the local department of social services, to investigate a self-reported incident regarding Care, protection and guidance. Provider interviews were conducted, documentation and photos were reviewed and classroom observations were made. Violations were issued as a result of this inspection. Violations:
Standard #: 22VAC40-185-340-A
Description: Based on review of documentation, photos, and staff interviews, the facility failed to ensure children's care and protection. Evidence: 1. On 12/19/19 at approximately 7:45 am, staff #1 left a slide close lid mug of coffee on the floor in the infant classroom while she went to greet an incoming child. Child #1 (age 10 months) knocked the mug over and began to play with the coffee that spilled on the floor. Child #1 was taken to an urgent care facility at approximately 7pm on 12/19/19 and diagnosed with epidurmal burns. 2. Three infants were observed sleeping in cribs that had sheets that were not tight fitting and allowed material to gather on the surface of the mattress presenting a suffocation or strangulation hazard. 3. One infant was observed sleeping with a bib and a pacifier with a strap, presenting a strangulation hazard.

Plan of Correction: Staff will be assigned trainings, CCEI900 Safety in the infant toddler classroom and CCEI119 Food allergies in early care settings. Staff is instructed that there is to be no drinks in the classroom except bottled water. Staff may have water in a clear sports bottle for water. No hot drinks are permitted in classroom. Management will monitor the classrooms for any violations. This plan of action will begin immediately with continuous monitoring. The staff will also be trained in SIDS and American pediatric safe sleep practices. These trainings will be completed within 30 days of 2/11/2020 staff meeting. Center will provide temporary bedding to infants if provided sheets do not meet the safe sleep standards. Staff will request parents to bring a replacement sheet.

Description: Based on observation, the facility failed to ensure all crib requirements were met. Evidence: two infants were observed sleeping in cribs that had the mattress propped up. There was more than 1" between the mattress and crib.

Plan of Correction: All staff working in the infant rooms will be assigned training through CCEI as stated above. The staff will also be trained in SIDS and American pediatric safe sleep practices. These trainings will be completed within 30 days of 2/11/2020 staff meeting.
