Technical Assistance:
Today we discussed the need for all board members background checks to be complete and current at the time of renewal.

Comments:
An unannounced monitoring inspection was conducted today from 9:30am ? 11:30am. There were five children in care ages 18 months through three years old with three staff. Children and staff records were reviewed. Required postings, annual inspections, emergency evacuation drills, first aid kit and medication were reviewed. A walking of the center inside and outside was completed. Children were observed engaged in teacher and self-directed activities including restroom routines, transitions hand washing, and inside play activities. Violations regarding staff records, annual inspections, physical plant and emergency items were identified and appear on the violation notice. The violations were discussed with the owner at the conclusion of the inspection.

Standard #: 22VAC40-185-180-D
Description: Based on record review and interview the center failed to demonstrate that drives have disclosed any moving traffic violation that occurred five years prior to or during employment or assignment as a driver.
Evidence:
Staff 1 stated that staff 2 drives the center's van to transport children to and from public school, however, the record for staff 2 does not contain documentation that staff disclosed information regarding any traffic violations.

Standard #: 22VAC40-185-280-B
Description: Based on observation the center failed to ensure that hazardous substances are kept in locked location using a safe locking method that prevents access by children.
Evidence:
1 - On the second floor of the center in the four year old classroom there are three chemicals stored in a cabinet that was unlocked.
a. The chemicals stored in the cabinet included clorox wipes, white board cleaner and lotion.
b. All container's labels state keep out of reach of children and caution, eye irritant.

Description: Based on observation and interview the center failed to ensure that there is a working battery-operated radio on each floor of the center.
Evidence:
Staff 1 stated that the radio on the second floor did not have batteries and therefore, was not operational.
