Technical assistance was provided regarding training.

Comments:
An unannounced renewal inspection was conducted today from :9:15am-2:00pm. There were 107 children (infants-5yrs.) supervised by 16 direct care staff. The physical plant,programming, 6 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, circle time,and music class. Diapering and hand-washing procedures were also observed. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 22VAC40-185-160-C
Description: Based on review of six staff records, one does not have documentation of an updated tuberculosis (TB) screening.
Evidence:
Staff #1, has documentation of an expired TB screening (1/29/20).

Plan of Correction: Staff will get the updated TB screening. We will review all records monthly.

Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, one is incomplete.
Evidence:
Child #1 does not have documentation of addresses for emergency contacts.

Plan of Correction: I will get the information, and review all records.

Standard #: 22VAC40-185-290-3
Description: Based on observation, not all electrical outlets are covered with protective covers.
Evidence:
Power strips in the 3 and 4 year old classrooms have 6 outlets not covered and are accessible to children.

Plan of Correction: Corrected during inspection, staff will check every morning for compliance.

Standard #: 22VAC40-185-320-B
Description: Based on observation, not all restrooms are equipped with toilet paper.
Evidence:
There is no toilet paper available for the children in the 3 year old classroom.

Plan of Correction: Toilet paper was replaced during the inspection. Staff will ensure compliance every morning.

Standard #: 22VAC40-185-370-5
Description: Based on observation and interview with staff, 30 minutes of tummy time is not being recorded daily.
Evidence:
Staff sends pictures of child during the tummy time without the amount of time documented.

Plan of Correction: Staff will begin recording the time.

Standard #: 22VAC40-185-570-E
Description: Based on observation, bottles located in the refrigerator are not labeled according to standards.
Evidence:
A bottle located in the infant room refrigerator was labeled with child's name, but no date.

Plan of Correction: Staff put the date on the bottle and will ensure compliance when accepting bottles from parents.
