Technical Assistance:
Consultation provided on the need to make sure the attendance in each classroom reflects the children that are present at any given time.
Outside time is required for infants- weather permitting.
Fire inspections must be up dated annually.
Closely monitor the diapering pads to ensure that they are not torn/ripped. If they are torn or ripped the expectation is that they be replaced immediately.
Once the mulch becomes compact and/or starts to degrade into dirt, it is no longer considered a resilient surface.

Comments:
An unannounced monitoring inspection was conducted today with the director. Twenty children were on site with four staff. During the walkthrough, the Licensing Inspector observed one of the two infants napping, the 2 year old children were making valentines day art, while the 3/4 year old children were observed outside on the playground.The director reported that there were no medications on site. A sample of six injury/accident reports were reviewed as well as six staff and children's files.

Standard #: 22VAC40-185-240-C
Description: Based on a review of six staff files and an interview with the director, it was determined that three staff had not obtained the 16 hours of annual training.
Evidence:
Staff A was hired on 5/18/18 and has 12 of the required 16 hours for ( 5/18 to 5/19).
Staff B was hired on 10/22/18 and had no documentation of annual training for 10/18 to 10/19.
Staff C has no annual training for 2018 to 2019.

Plan of Correction: We will locate the training certificates and if needed provide additional training.

Standard #: 22VAC40-185-270-A
Description: Based on observation, it was determined that the center is not operating in a clean, safe or operable condition.
Evidence:
1) The Licensing Inspector (LI) observed a red tricycle missing the hand grips on the handle bars, which presents a finger entrapment hazard;
2) The LI observed two pieces of green outside carpeting placed on top of tree roots on the playground, the carpet was bunched up and present a tripping hazard;
3) The LI observed a table ( without the legs) laying flat on the playground. The LI observed children jumping on the table;
4) The LI observed four loose- fitting sheets in the cribs for the 4 infants enrolled in that classroom;
5) Interviews and observation revealed that the step on container used to dispose of soiled diapers is located in the hallway, which is outside of the infant room. After changing an infant, the teacher opens the classroom door and disposes of the diaper, consequently the door handle is being cross-contaminated;

Plan of Correction: We will move the step on container in the classroom today. We will remove the tricycle until we can get the rubber grips, we will remove the table from the playground today, we will make sure the carpet on the playground used to cover the roots is not bunches and caused a tripping hazards today. We will replace the crib sheets to ensure that they are tight-fitted .by 02/18/2020

Standard #: 22VAC40-185-330-A
Description: Based on observation, it was determined that the fall zone of the play equipment on the playground does not have at least six inches of resilient surfacing nor does it have at least 6 feet of fall zone.
Evidence:
The green and blue portable slide on the playground was not located under any resilient surfacing, six inches of resilient surfacing is required..
The green and blue portable slide was within 2 feet of the chain link fence and not the required 6 foot fall zone.

Plan of Correction: We will remove the slide until we can add the appropriate amount of mulch.
