Comments:
2/4/2020 - Site visit for renewal. A Subsidy Health and Safety inspection was conducted on this date to review supplemental health and safety requirements. Licensing Inspector arrived at 3:40 and departed at 4:45 to conduct the site visit for the renewal. At the time of the inspection, there were 17 school-age children in care with two staff. Children were observed playing in the gym and eating lunch. There was no medication be administered or stored on site. The sample size was five children's records. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

2/12/2020 - Record review for renewal conducted at AlphaBest office. The sample size was three records.

Violations:
Standard #: 22VAC40-665-500-F
Description: Based on review of documents at the facility, it was determined that the facility did not maintain a current written list of all children's allergies, sensitivities and dietary restriction.

Evidence:
The allergy list available was dated 9/3/19. Staff stated that there were children on the list that were no longer enrolled and children with allergies who were not on the list.

Plan of Correction: All site directors will now print out new allergy lists on the first of every month to ensure that it is kept current.
