Comments:
An unannounced monitoring inspection was conducted today from 3:00pm-4:45pm. There were 67 children (5-12yrs.) supervised by 6 direct care staff. The physical plant, 3 staff records, 8 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed checking into class, eating snack, reading books, creating rubber band bracelets, playing with Legos, drawing, and creating a bridge using popsicle sticks. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Description: Based on record review, the center did not maintain the following information for each child:

Evidence:
1. Child #1's (age: 6 years old) record did not contain documentation of the physician's phone number, and immunization and physical records signed by a physician.
2. Child #6's (age: 9 years old) record did not contain documentation of information for a second emergency contact person.

Plan of Correction: I will speak to the parent today and get the missing information.
