Licensing Inspector arrived at 6:30 and departed at 7:40 AM to conduct the site visit as a part of the renewal. At the conclusion of the inspection, there were 24 school-age children in care with three staff. Children were observed participating in center based activities and then doing exercises in the gym. There was no medication being administered or stored on site. The sample size was five children's records.
Record review for renewal conducted at AlphaBest office. The sample size was three records.
Standard #: 22VAC40-185-60-A Description: Based on review of five children's records, it was determined that the facility did not ensure that all required information is present in each child's record.
1. The record for child #3 (date of enrollment 9/3/19) did not include addresses for either of the two required emergency contact persons.
2. The records for child #4 (date of enrollment 9/3/19) and child #5 (date of enrollment 9/11/19) did not include an address for one of the two required emergency contact persons.
Plan of Correction: Center management will work with parents to obtain addresses and will continue to audit children's records to ensure that they are complete.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of three staff records, it was determined that the facility did not ensure that each staff had a completed search of the central registry on file within 30 days of employment.
The record for staff #2 (date of hire 10/22/19) did not contain a completed search of the central registry.
Plan of Correction: Management will follow-up with Office of Background Investigations and submit a new check if required.
