Technical Assistance:
Technical assistance today included storage of medications and physical environment.

Comments:
An unannounced renewal inspection was conducted this afternoon from 3:45 - 5:10 pm. There were 52 children present at the beginning of the inspection with four staff. I observed children practicing for a school talent show with dance moves and music; stacking blocks to create an intricate building; involved in dramatic play; playing ping pong; playing a card game and drawing.Children's files, staff records and medications were well-organized. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and staff confirmation during today's inspection, some areas and equipment were not maintained in a clean, safe and operable condition.
Evidence:
The large blue area rug had been obviously been taped to the floor numerous times, but it still did not lay flat, with several areas of bunching in the middle and loose at the edges, posing a tripping hazard. Two smaller mat rugs were overlapping near the door, also posing a tripping hazard and appearing grimy. The couch had big gashes in the surface and were overed with tape.

Plan of Correction: We have tried to keep the rug flattened. We will consider removing it and asking for a replacement. The smaller rugs can be cleaned or replaced. The couch will be replaced.

Description: Based on a review of records today, one medication authorization did not contain all required information.
Evidence:
The medication authorization for Child # 2 had a physician's signature but no parent signature as required.

Plan of Correction: We will talk to the parent today.

Standard #: 22VAC40-185-550-D
Description: Based on a review of records today, there were not two emergency shelter-in-place drills documented as required.
Evidence:
The last documented shelter- in- place drill was during the last school year - 4/19.

Plan of Correction: We will conduct two shelter-in-place drills this school year.
