Comments:
An unannounced monitoring inspection was conducted on February 12, 2020. At the time of entrance there were 123 children and 19 staff members present with children. Additional staff were on site for administrative and meal preparation responsibilities. The sample size consisted of 12 children's records and 7 staff records. Children were observed reading books, going outside eating lunch, freely playing and working on Valentine projects. The inspection began at approximately 10:15 am and ended at approximately 1:15 pm. Areas of non-compliance are identified on the violation notice. The results of the inspection and violations cited were reviewed and verified with the center director during the exit interview.
Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, and then return it to the licensing office within 5 calendar days of receipt.

Violations:
Standard #: 22VAC40-185-260-B
Description: Based on record review , the licensee did not ensure there was a yearly health inspection conducted.

Evidence:
The most recent health inspection was conducted 1/10/2019 which is more than a year old.

Plan of Correction: 260-B: The health department was contacted for a date during the visit. Moving forward, the management team will calendar all permit expirations 30-45 days in advance to ensure compliance.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the licensee did not ensure areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence:
In the toddler 2 classroom, there was a plastic frame on the window sill that was cracking with sharp edges.

Plan of Correction: 270-A: We are working in conjunction with our facilities team to ensure that the window sill is repaired on or before 3/13/20. Our safety captain will check all windows monthly to ensure that we remain in compliance with this.

Standard #: 22VAC40-185-500-A
Description: Based on observation , the licensee did not ensure the center Staff wash their hands with soap and running water before and after helping a child use the toilet or a diaper change.

Evidence:
In the toddler 1 room, Staff#1 did not wash her hands after diapering.

Plan of Correction: Toddler one staff will all be retrained on proper diapering procedures on or before 2/17/20. In addition, management will observe weekly for compliance.
