Comments:
An unannounced Monitoring Inspection was conducted on February 6, 2020, from approximately 12:40 pm until 2:40 pm. At the time of entrance there were 51 children and 8 staff members present with children. There were additional staff on site for kitchen and administrative support. The sample size consisted of 6 children's records and 5 staff records. Children were napping and eating snack. Previous violations were reviewed. Areas of non-compliance are identified on the violation notice.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review, the licensee did not ensure the staff resubmit TB results every 2 years.

Evidence: Staff #2 most recent TB test was 11/28/17. The TB test was more than a two years old.

Plan of Correction: Director will ensure that all staff have updated TB tests

Standard #: 22VAC40-185-40-J
Description: Based on record review and staff interview, the licensee did not ensure the injury prevention plan was updated annually.

Evidence:
The center director confirmed the injury prevention plan was not updated annually.

Plan of Correction: Director will prepare an injury prevention plan annually.

Standard #: 22VAC40-185-280-B
Description: Based on observation and staff interview, the licensee did not ensure the hazardous substances were kept in a locked place.

Evidence:
In the girls restroom, there was diapering cream on a the shelf . The diapering cream was labeled "Keep out of reach of children" and "Warning".

Plan of Correction: Director will ensure that all hazardous substances are kept in locked places out of reach of children. Director will also retrain staff.

Standard #: 22VAC40-185-380-A
Description: Based on observation and staff interview , the licensee did not ensure there was a posted schedule for each class.

Evidence:
In the Pre-K-1 classroom, there was not a daily schedule posted. Confirmed with the director.

Plan of Correction: Director will ensure that all classroom have posted daily schedule.

Standard #: 22VAC40-185-500-B
Description: Based on observation and staff interview, the licensee did not ensure the staff disposed diaper in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Evidence: There was two observation of staff touching an exterior surface the door knob to dispose of the diaper in the foot operated storage system in the bathroom. The center director confirmed the process of the disposal of the diaper.

Plan of Correction: Director will retrain staff over diaper changing procedures

Standard #: 22VAC40-185-550-D
Description: Based on staff interview and documentation, the licensee did not ensure there was two shelter in place drill completed annually.

Evidence: The director confirmed there was only one shelter in place drill completed during 2019.

Plan of Correction: Director will ensure that there are a minimum of 2 shelter in place drills per year
