Technical Assistance:
The licensing inspector and administration discussed Standard 22VAC40-185-500B; to ensure the diapering area shall be provided with a sink with warm running water. Warm running water is defined as "water between 60?F and no more than 120?F."

Comments:
An unannounced monitoring inspection was conducted on February 7, 2020 from approximately 3:25 PM to 6:00 PM to determine the center's compliance with licensing standards. This visit focused on the main areas of previous non-compliance. Observations were made, interviews were conducted, and records were reviewed. The following staff to child ratios were observed: Infants 1:3, Toddlers to School Age group 2:15.

Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the licensing office within 5 business days. Your plan of correction should contain 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Standard #: 22VAC40-185-70-A
Description: Based on review of four staff records, two records did not contain the required information. Evidence: 1. The licensing inspector observed the records for Staff #2 (hire date 1/22/20) and Staff #4 (hire date 1/21/20) did not contain documentation of two references prior to employment and did not contain the required information about any health problems which may interfere with fulfilling the job responsibilities.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure all hazardous substances were kept in a locked place. Evidence: The licensing inspector observed a container of bleach water, a bottle of spray disinfectant, and a bottle of air freshener on a shelf in the first bathroom used by children. A bottle of spray disinfectant was observed on a shelf in the second bathroom used by children.

Description: Based on observation, the licensee failed to ensure that the sinks in restrooms used by children are supplied with water that does not exceed 120 degrees Fahrenheit. Evidence: The temperature of the water from one of two sinks, in the restrooms used by children, was measured and was observed to be 123.8 degrees Fahrenheit.

Description: Based on observation, a record review, and staff interview, the center did not ensure that staff to children ratio requirements applicable to the youngest child in the group were followed when children were regularly in mixed age groups. Evidence: 1. The licensing inspector observed a staff to child ratio of 2:15 in the main classroom, in which there were children in care with ages ranging from toddler to school age. 2. The record for Child #4 (DOB 2/17/18) indicated the child was 23 months old. 3. Administration and staff reported Child #4 is regularly in care with the group in the main classroom, and had moved into the classroom in January 2020. When children are regularly in ongoing mixed aged groups, the staff-to-children ratio applicable to the youngest child in the group shall apply to the entire group. The ratio should have been 1:5.

Standard #: 22VAC40-185-410-2
Description: Based on observation and an interview, the licensee failed to ensure that a child is not enclosed in a small confined space or any space that the child cannot freely exit himself. Evidence: 1. The licensing inspector observed Child #4 (23 months old) crying and trying to climb out of a high chair. 2. An interview with administration indicated that Child #4 was in timeout. Administration reported the child would not sit still for a timeout, and this is why the highchair was used. 2. Administration reported children are placed in timeout for one minute per the child's age. 3. The high chair was not being used for the intended purposes when Child #4 was placed in it to manage his behaviors.
