Comments:
An unannounced renewal inspection was conducted on February 10, 2020 from approximately 11:55 AM to 3:50 PM to determine the center's compliance with licensing standards. The following staff-to-child ratios were observed in the classrooms: Infants 2:6, Toddlers 1:5, 2 year olds 2:13, 3 year olds 1:8, 4 year olds 2:17. Lunch today consisted of chick peas with brown rice, peas and carrots, pears and milk. The staff were observed to be engaged with the children in age appropriate activities and to be meeting the needs of the children in care. The facility had the required postings in the facility. The first aid kit and non-medical emergency supplies were inspected and found complete. The last evacuation drill was conducted on 1/3/20. The last shelter-in-place drill was conducted on 1/3/20. The Emergency Preparedness plan is current. Health Inspection: 8/1/19. Fire Inspection: 11/25/19. Medications and authorizations were reviewed. There were staff present who are certified in medication administration, daily health observation, First Aid, and CPR. 5 children's records and 5 staff records were reviewed. Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure all areas and equipment were maintained in a clean, safe, and operable condition. Evidence: 1. In the school age room, the licensing inspector observed unclean conditions on the floors near the sink and on the wall behind the sink and trash can. 2. On the buses, the licensing inspector observed trash on the floors. 3. In the 2 year old class, the licensing inspector observed a torn screen in a window which was open during the inspection.

Plan of Correction: Administration will ensure the repairs are completed and the cleaning needs are completed daily.

Description: Based on observation, the center did not ensure that medication shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: The licensing inspector observed packets of unlocked non-prescription medications in the first aid kit on a bus.

Plan of Correction: Corrected on site.

Standard #: 22VAC40-185-580-C
Description: Based on observation, the center did not ensure that the required information was present in vehicles used for the transportation of children. Evidence: 1. The licensing inspector observed that Bus #1, directly following the transportation of children, did not contain emergency numbers, the center's address, or the center's phone number. 2. Prior to the transportation of children, the licensing inspector observed that Bus #2 did not contain the center's address or phone number. Bus #2 contained contact information for another location.

Plan of Correction: Administration will ensure all the required information is present in vehicles whenever children are transported.
