Comments:
A Subsidy Health and Safety inspection was conducted on February 10, 2020 from approximately 11:55 AM to 3:50 PM to review supplemental health and safety requirements. The following staff-to-child ratios were observed in the classrooms: Infants 2:6, Toddlers 1:5, 2 year olds 2:13, 3 year olds 1:8, 4 year olds 2:17. The staff were observed to be engaged with the children in age appropriate activities and to be meeting the needs of the children in care. The facility had the required postings in the facility. There were staff present who are certified in medication administration, daily health observation, First Aid, and CPR. 5 children's records and 5 staff records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Standard #: 22VAC40-665-540-A
Description: Based on a review of five staff records, two records indicate the center did not ensure staff shall submit documentation of a negative tuberculosis (TB) screening prior to coming into contact with children, which shall be no more than 30 days old at employment. Evidence: 1. The record for Staff #3 (hire date 1/23/20) contained documentation of a TB screening dated 3/25/19. 2. The record for Staff #5 (hire date 1/6/20) contained documentation of a TB screening dated 8/9/19.

Plan of Correction: Administration will ensure that TB screens are completed in the required time frames.
