A unannounced renewal was conducted with the director and staff. 39 children were in care during the inspection and the age range of the children was eight months to five years of age. Description: Based on observation of the playground surfacing it has become worn out in areas and splitting at the seams.
Evidence: The surfacing on the school age playground is separating at the seams and posing a tripping hazard to the children.

Plan of Correction: We will call maintenance.
