Comments:
An unannounced Complaint Inspection was initiated in the center on January 21,2020, continued on February 3, 2020 and completed on February 10, 2020 regarding a complaint received by the department related to Physical Plant, Staffing/Supervision, Special Care Provisions/Emergencies and Special Services. Based on observation and interviews with staff and other parties involved, there is a preponderance of evidence to support the allegation regarding Physical Plant, therefore the complaint has been determined to be VALID.
Violations not related to the compliant were cited and noted on the violation notice.

The results of the inspection were reviewed and verified with the center director during the exit interview.

Standard #: 22VAC40-185-270-A
Complaint related: Yes
Description: Based on observation and staff interviews, the licensee did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence: On January 21, 2020, the following physical plant items were observed and confirmed wit the center director on January 21, 2020.
1) In classroom #5, near the sink there are 3 screws protruding from the wall and the paint is chipping and peeling and 2 out of 2 window frames have areas of chipped and peeling paint.
2) In classroom #4, 1 out of 2 window frames have chipped and peeling paint.
3) In classroom #2, near the entrance door there are 2 screws protruding form the wall and 1 out of 3 window frames have chipped and peeling paint.
4) In classroom #7, there are 3 screws protruding from the shelf around the window, 2 screws protruding from the wall near the girls restroom sink, 2 screws out of the wall near the boys restroom and 3 out of 5 windows have chipped and peeling paint around the window frames.

Plan of Correction: Repairs were completed 1/31/2020.

Complaint related: No
Description: Based on observation and staff interviews, the licensee did not ensure that children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.

Evidence:
On January 21, 2020, in room #7, 1 child was observed eating snack and in classroom #2, 4 children were observed eating snack and none of them washed their hands after eating. On February 3, 2020, in classroom #7, 18 out of 18 children were observed eating snack and they did not wash their hands after eating.

Plan of Correction: Staff will be retrained.

Complaint related: No
Description: Based on observation and staff interviews, the licensee did not ensure children three years of age or younger may not be offered foods that are considered to be potential choking hazards.

Evidence: In classroom #2, children ages 16 months to age 2 were served apple slices that measure between 1/4 inch to 1/2 inch in width and 1/2 inch to 1 inch in length.

Plan of Correction: We will ensure all fresh fruit is diced smaller.

Complaint related: No
Description: Based on observations and staff interviews, the licensee did not ensure that no child shall be allowed to drink or eat while walking around.

Evidence: On January 21, 2020, in classroom #2, 2 children were observed walking around and eating during morning snack (apples).

Plan of Correction: Staff will be retrained.
