Comments:
An unannounced renewal inspection was conducted from 9:15 - 11:30am There were 27 children in care with six staff. Children and staff records were reviewed. A walking tour of the center inside and outside was completed. Children were observed in teacher and self-directed activities including morning snack and play activities. Violations regarding physical plant, medication, and food were identified and appear on the violation notice. The violations were reviewed with the person in charge of the center at the conclusion of the renewal inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure that equipment is maintained in operable condition.
Evidence:
1 - On the playground there are two pieces of ride on play equipment with areas of cracked and or slitting plastic, a cozy coup and a motor cycle.
a. The cracked plastic leaves jagged edges. The splitting plastic creates pinch points with jagged edges.
2 - There are nine ride on motor cycles that are missing the handle bars leaving two open circles (holes) in the plastic of the toys.
a. The holes have rough edges, are large enough to two to three fingers to fit in, and could lead to injury.

Description: Based on observations the center failed to ensure that electrical outlets have protective covers.
Evidence:
There was not an outlet cover on one outlet on the playground.

Description: Based on observation the center failed to ensure that medication is stored in a locked place using a safe locking method that prevents access by children.
Evidence:
1 - There were medications stored in a cabinet in the school age classroom that does not prevent access by children.
a. The cabinet was sitting on the floor and within reach of children.
b. The latch on the cabinet is taped on one side and requires a single push of a button to open the the latch.
c. There were two prescription medications stored in the cabinet.
