Medication listed as an action in case of an allergic exposure should be on-site. If the parent does not bring the medication into the center the parent will need to modify the "action to take".

An unannounced monitoring visit was conducted today. Observed children reading with special visitors from the local high school, transitioning to hand washing and snack (from home) a birthday celebration and free play. Reviewed a sample of records for children, staff trainings, medication authorizations and storage, physical space and care of children.Areas of non-compliance are identified in the violation notice. Evidence:
Child #2's physical was incomplete within 30 days of enrollment. (date of enrollment 8/2019)Plan of Correction: Staff gave parent missing physical page and asked them to have it completed on 1/24 and 1/31
Standard #: 22VAC40-185-60-A Description: Based on record review, 2 of 3 children's records did not contain all required information.
Evidence:
1. Child #1 was missing 2 of 2 emergency contact phone numbers.
2. Child #3 was missing a @nd emergency contact name, address and phone number.Plan of Correction: Staff followed up with parents and received missing information.
Standard #: 22VAC40-185-260-A Description: Based on observation, the annual fire permit was expired.
The posted fire permit expired 11/2019.Plan of Correction: Fire inspection was completed on 1/24/2020 waiting for permit to post.
Standard #: 22VAC40-185-270-A Description: Based on observation, an area of the center is not maintained in a clean, safe or operable manner.
Evidence:.
The wall between the bathroom and room sink has several small spots of peeling paint.Plan of Correction: Staff spoke to facility manager on 1/24/2020 . Administrative staff will follow-up with facility manager.
Standard #: 22VAC40-185-510-C Description: Based on medication review, the center did not follow the procedures for maintaining medication on--site.
Evidence:
Medication for child #4 was not in a labeled box with an identifying label, the emergency medication expired 10/2018 and the center did not dispose of the medication within 14 days of the child's last day of enrollment 8/2019.Plan of Correction: Staff were unable to get in touch with parent/guardian. Staff disposed of expired epi-pen.
