Comments:

A Subsidy Health and Safety inspection was conducted on February 18, 2020 to review supplemental health and safety requirements. The inspection was initiated at 11:00 am and concluded at 2:00 pm. Upon the inspector's arrival, preschool children and the two year old class were outside on the playground. Children played on small trikes, in the sand box and in the playhouse. In the toddler class children were playing with animal puppets and a race track with cars. Staff participated in play making animal sounds with the children and racing the cars. Infants were observed having play time on the floor with books and toys. Later children listened to stories and prepared for lunch. Bathroom breaks and hand washing was conducted. Today's lunch consisted of tomato soup, grilled cheese sandwich, pears and milk. Nap time was also observed. All areas of the center to include the playground area were inspected. The center has age appropriate materials and equipment. The first aid kit and non-medical emergency supplies were reviewed and found complete. Required posted documentation was reviewed. The inspector interacted with staff and children in care. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Five children's records and five staff records were reviewed. Staff on site have current CPR, First aid, DHO and MAT certification. Per the director, currently medication is not being administered and is not on site. The most recent evacuation drill was documented on 02/04/2020. The two required shelter-in-place drills for 2019 were completed on 03/19/2019 and 10/17/2019. A lock down drill was conducted on 09/18/2019. Today's Census: Infants: 4 infants with 1 staff. Toddler: 8 children with 2 staff. Two's: 11 children with 2 staff. Pre-school 3-5 years: 14 children with 2 staff. Description:	Based on review of staff records, the center did not obtain documentation of a tuberculosis screening at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. Evidence: Staff #1, with a hire date of 12/2/2019, had documentation of a tuberculosis screening dated 12/06/2019, exceeding the date of employment.
