Comments:
An unannounced inspection was completed on 02/11/2020 from 9:10 A.M.-1:00 P.M. At the time of the inspection 77 children were in care with twelve staff and two administrators present. Eight children records, eight staff records, two board members background checks, three medications and required documentation, fire inspection, health inspection, over the counter topical ointments and required documentation, six injury reports, emergency evacuations, first aid kits, emergency supplies, parent handbook, and posted required information were reviewed. The playgrounds were not inspected due to inclement weather. Children were observed during AM snack, handwashing, diapering, arrival, transition to circle time, music and movement activities, free choice activities that included sensory activities with slime, and the sensory table. Children were also observed completing literacy activities which included letter recognition, lunchtime, naptime, and teachers completing mid-year assessments on children were observed. The exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-185-420-A
Description: Based on review of center documentation and interviews with administrative staff, the center failed to provide parents with all required information in writing before the child's first day of attendance.

Evidence:
1. The center's parent handbook was missing the following information: policy for application of sunscreen, diaper ointment, and insect repellent; established lines of authority; and the custodial parent's right to be admitted to the center.
2. Administrative staff verified that the parents were not provided with all required information in writing before the child's first day of attendance. Some of the information was found in the handbook that is given to children who enroll in the summer camp program, but could not be located in the handbook given to the parents of children enrolled in the pre-school program.

Plan of Correction: Administration will review the current handbook and add the missing information.

Standard #: 22VAC40-185-510-E
Description: Based on observation of medication, the center failed to ensure that medication shall be labeled with the child's name, dosage amount, and times to be given.

Evidence:
1. In the Upper Building there was a bottle of Children's Allergy medication that was not labeled with the child's name, dosage amount, or times to be given.
2. Staff verified that the medication was not labeled with child's name, dosage amount, and times to be given.

Plan of Correction: Administration immediately labeled the medication with the required information. In the future the administrator at each building will check all medications at intake to ensure all required information is obtained.
